Pre-Interview Membership Form

Thank you for your interest in our church and the decision you have made to pursue membership in this church. I believe that church membership is an important step for all those who are committed to following Christ.
Included with this letter you will find the following:

  1. Pre-Interview Form
  2. Responsibilities of church membership
  3. Church survey

Please fill out the interview form and return it to the church office. I will then schedule a meeting with the Session. Please include a phone number where you may be reached for this scheduling.
We pray that this step will provide fellowship and spiritual guidance for you.

Pre-Interview Form

1. Do you have confidence that if you were to die today, you would enter Heaven?

2. If you were to die today and stand before God and He asked you, “Why should I let you into my Kingdom?” what would you say?

3. Describe your relationship with and experience of God until now, including how you came to know Him.

4. Have you been baptized? If so, please provide the aproximate date and the name of the denomination in which you were baptized.

5. Have you ever been a member of a PCA Church?

6. If “yes” to the above, are you currently a member in good standing of a PCA Church? Also, please provide Name & Address.

7. If “yes” to #5 & “no” to #6, why did the status of your membership change?

8. Are you currently a member of any church?


Responsibilites of Church Membership

Church membership is not an option for Christians. It is a requirement because it is impossible to obey many of the commands of the New Testament apart from an official and formal association with, and commitment and submission to, a visible body of Christians. These commands include the use of spiritual gifts as a constituent part of a full-orbed ministry, according to the figure of different parts of a body contributing different things to the needs of the whole (I Corinthians 12-14), and submission to church officers (Hebrews 13:17; I Peter 5:1-5).

The following questions summarize officially the understanding of the Presbyterian Church in America concerning the qualifications and duties of membership in a particular church expression of the visible community of God’s covenant people. You should read and consider them carefully, as they will be the questions presented to you in the formal service in which you are received as a member. You will answer them as part of a public profession of faith, and in doing so you will be entering into a solemn covenant with God and His Church.

Questions for Prospective Members:

  1. Do you acknowledge yourselves to be sinners in the sight of God, justly deserving His displeasure, and without hope save in His sovereign mercy?

  2. Do you believe in the Lord Jesus Christ as the Son of God and Savior of sinners, and do you receive and rest upon him alone for salvation as He is offered in the Gospel?

  3. Do you now resolve and promise, in humble reliance upon the grace of the Holy Spirit, that you will endeavor to live as becomes the followers of Christ?

  4. Do you promise to support the church in its worship and work to the best of your ability?

  5. Do you submit yourselves to the government and discipline of the church, and promise to study its purity and peace?
